You can use the Plan Finder and view results without submitting contact details. Your answers stay in the current page unless you ask for your suggestions by email.
When you choose to stay in touch
We save your first name, email address, mobile number if provided, how you reached the form, your selected membership if you make an inquiry, and a dated record of your contact choices. If you request a plan email, we also save the suggested treatment names and the educational email prepared for you. We do not ask for medical history, photographs, medication lists, or payment details in this form.
Plan emails and updates
A plan email is a one-time request for the suggestions shown to you. Email marketing, text marketing, and membership inquiries have separate optional selections. Choosing one does not select the others. A plan request is not membership enrollment, a medical consultation, or confirmation of treatment eligibility.
Once automatic email is enabled, our email delivery provider processes your email address and requested message. If delivery is unavailable or fails, the page tells you; your request remains saved for Aria to follow up.
Shared festival iPads
Please select “Next guest” when finished. Shared iPad mode clears the screen after three minutes of inactivity. Contact details and answers are not saved in local browser storage. A reset clears the screen; it does not delete a request already submitted to Aria.
Change your choices
To update contact permissions or request deletion of your guide submission, contact concierge@ariasonata.com. Use the unsubscribe option in marketing emails or reply STOP to marketing texts you receive. You can decline marketing and still explore treatment options or purchase services.
